Your team investment fee includes practice uniform, a shooting shirt and travel to local games/tournaments in Illinois. The player is responsible for all additional expenses, including travel for out of state games/tournaments.

2. Scan and email the (1) application, (2) release form, (3) and payment plan form if you’re using one to info@chicagotransformers.com, or fax to 630-455-7495.

3. If paying by check or money order, payment must be RECEIVED in full with the application or by the scheduled dates in the payment plan form. Mail to:
